Job Opportunity:

Under the direct supervision of the Program Manager, the Case Manager will be responsible for the coordination and monitoring of a variety of services to meet the often complex and competing needs of homeless Veterans and their families.  This includes completing initial and ongoing assessments, developing and implementing client-defined, goal-oriented Service Plans, assisting clients to obtain and maintain housing, providing support and education, and working as part of a team to ensure client needs are met.  The Case Manager will work closely with other Volunteers of America staff and other providers to efficiently coordinate referrals and identify access to services.

Job Duties:

1. Complete thorough intake and eligibility screening with referred clients.
2. Coordinate referrals from Outreach/Intake staff and inform team of capacity status.
3. Assess client's housing needs and strengths and supports available to client.
4. Develop comprehensive Service Plan with clients and family.
5. Participate in rapid re-housing program team meetings and engagements to develop and monitor Service Plan.
6. Provide Outreach services to social service providers and other community organizations who serve homeless individuals and families as needed.
7. Educate homeless service providers on program criteria, build and maintain these relationships, and provide additional support as needed.
8. Use a time-sensitive approach to determine eligibility of potential veteran participants.
9. Refer ineligible participants to appropriate community resources including service providers.
10. Conduct home visits, escorts, and other related activities to engage families at-risk for homelessness.
11. Enter information into Homeless Management Information System upon intake and participate in local CAHP system.
12. Maintain complete and comprehensive client file including case notes and Service Plan in electronic database.
13. Educate homeless providers and the general community about the program through informational events and community meetings.
14. Develop and maintain knowledge of community resources including but not limited to Veteran-friendly employment agencies, vocational training programs, and local employment agencies.
15. Establish and maintain ongoing relationships with community service providers.
16. Meet regularly with program team to coordinate services and ensure continuum of care.
17. Maintain a well organized and clean work environment
18. Assists with planning program events and activities that inform Veterans, Volunteers of America staff, and Community Groups about program services.
19. Attend appropriate workshops and opportunities to continually improve knowledge and skills.
20. Attend meetings as required by HUD and CoC.
21. Maintains strict confidentiality at all times in communications.
22. Maintains a professional plans for self and direct reports.
23. Other tasks as assigned.

Job Qualifications:

Bachelor's Degree in social services or related discipline. Minimum of three year's experience in case management or equivalent position with experience working with veterans and/or other homeless populations preferred. Ability to successfully pass fingerprint and criminal investigation screening. Must have valid driver's license and meet insurance guidelines established by the Corporation. Ability to have a TB test and health screening. Knowledge of programs criteria and applicable community resources. Demonstrated ability to apply independent judgment in critical situations involving Veterans and their families.

  1. Must have ability to read, write and speak in English. Bilingual in Spanish helpful.
  2. Must be proficient with computer systems and software, including Microsoft Word, Excel, Outlook, and ServicePoint.
  3. Must have demonstrated ability to understand and articulate best practices around services for at-risk individuals and families.
  4. Must have demonstrated ability to build rapport and gain trust with diverse individuals.
  5. Must have working knowledge of community resources in San Diego County.
  6. Must have working knowledge of VA and other mainstream resources.
  7. Must have demonstrated ability to build and maintain partnerships with community providers.
  8. Must have demonstrated ability to work independently and as a team.
